Documents Required for J2 Visa The documents required for J-2 visa are: Completed USCIS Form I-765, Application for Employment Authorization, and signature card (available at OIE) . Check or money order in the amount of $100 made payable to the USCIS . Photocopy of USCIS Form I-94 (front and back) of J-1 (student or scholar) . Photocopy of USCIS Form I-94 (front and back) of J-2 (dependent) . Photocopy of current Form DS-2019 Certificate of Eligibility . Cover letter . Photocopy of first three pages in passport (clear copy of passport and/or visa photo required) . Two photos with a white background taken no earlier than 30 days before submission to the USCIS .
